Metformin

Metformin is used in
type II diabetes and in the
management of PCOS (unlicenced).
It should not be commenced if the patient’s eGFR is less than 30 ml/min/1.73 m2 and it should be withdrawn in acute myocardial infarctions.
Metformin side effects
Side effects include abdominal pain, diarrhoea and nausea. Metformin may cause a decrease in vitamin B12 absorption.
LAST treatment
In a patient who is not in cardiorespiratory arrest
usually treatment should be given for hypotension and any arrhythmias.
However, the definitive treatment is intravenous 20% lipid emulsion.
This is given as a bolus of 1.5 ml/kg followed by an infusion at 15 ml/kg/hour.
A maximal dose of 12 ml/kg may be given.
