A

from megakaryocytes in the bone marrow which differentiate from hematopoietic stem cells

Megakaryocytes lose their ability duplicate DNA by mitosis and this leads to a polyploid megakaryocyte

the cytoplasm matures and becomes granular which forms a platelet
