Vancomycin timing
A
IV over an hour, ideally should be over 2hrs ahead of time.

A
Amoxicillin 2gms, P.O.
Ampicillin, 2gms, IV
If Penicillin allergic:
Ceftriaxone 1gm IV
Cefazolin 1 gm IV (not in pt hypersensitive to PCN)

1g mixed in 250ml saline

- Flu like syndromes

- rashes, alopecia
- changes in CV, thyroid, hepatic function
