Antihistamine drugs Flashcards
Potency of Levocetirizine, fexofenadine, desloratidine
Potency: D>Levo>F
Hydroxyzine PAMoate
1st gen H1 blocker
- insomnia, anxiety (crosses BBB better)
A for anxiety
Cyclizine
1st gen. H1 blocker
- motion sickness, vertigo
Brompheniramine
1st gen H1 blocker
- Allergies
- Moderatly sedating
- Often found in combination with “cold” products
- avoid in elders
Azelastine
2nd gen. H1 blocker
- Dual MOA
- mast cell stabilizer and H1 blocker
- application to cause may cause nose bleed but less systemtic side effects
- formulation=nasal spray
Fexofenadine
2nd gen. H1 blocker
- highest safety profile (no cyp metabolism)
Nizatidine
H2 blocker
Ranitidine
H2 blocker
Cimetidine
H2 blocker
Chlorpheniramine
1st gen H1 blocker
- Allergies
- Moderatly sedating
- Often found in combination with “cold” products
- avoid in elders
Famotidine
H2 blocker
Cyproheptadine
1st gen. H1 blocker
- weight gain due to 5-HT2 blockade
Doxylamine
1st gen. H1 blocker
- Used for: Insomnia & allergies
- Highly sedating
- large doses= QTc prolongation
-
Avoid in elders
- strong anticholinergic effects
Meclizine
1st gen. H1 blocker
Motion sickness vertigo
Hydroxyzine HCL
1st gen H1 blocker
- Allergies, urticaria
Olopatidine
2nd gen. H1 blocker
- Dual MOA
- mast cell stabilizer and H1 blocker
- application to cause may cause nose bleed but less systemtic side effects
- formulation=nasal spray
Levocetirizine
2nd gen. H1 blocker
Epinephrine
histamine antagonist
- relaxes smooth muscles
- stimulates alpha and beta receptors
Diphenhydramine
1st gen. H1 blocker
- Used for: Insomnia & allergies
- Highly sedating
- large doses= QTc prolongation
- Avoid in elders
- strong anticholinergic effects
Cromolyn sodium
histamine antagonist
- works over several weeks
- stabilizes mast cells to prevent degranulation
- used as adjunct for pts. with severe allergies or allergic asthma
Desloratidine
2nd gen. H1 blocker
Least likely to cause sleepyness
Loratidine
2nd gen. H1 blocker
- Many CYP DDI’s
Promethazine
1st gen. H1 blocker
- Anti-emetic (E for anti-Emetic)
- highly sedating
- alpha blockade→hypotension
- D2 blockade→dystonic rxn’s, akathisia (inability to sit still)
- Strong anti-cholinergic effects
Cetirizine
2nd gen. H1 blocker
- causes the most sleepiness
