Antihelminths Flashcards
Benzamidizole mechanism and resistance
Mechanism:
- inhibits micro tubule polymerization
- inhibits energy production
Resistance: switch beta tubulin isotope, beta tubulin point mutation
Benzimidazoles (bug and stage)
- Nematodes (but also works for certain Cestodes and trematodes)
- eggs, larvae, and adults
Ivermectin (bug and stage)
- Nematodes
- microfiliarie
Specifically indicated for onchocerciasis (combo w/ steroid), and lymph filariasis (combo with benzimidazole)
Ivermectin contraindications
Loiasis
Ivermectin SE
Mazzoti rxn (use steroids)
Benzimidazole SE
GI, bone marrow suppression, HA
Ivermectin mechanism (and resistance)
Mechanism:
Activated glutamate-gated Cl- channels, causing flaccid paralysis
Resistance:
P- glycoprotein, mutation in channel
DEC indications (bug and stage)
- Nematodes
- microfilariae, adults
- specifically indicated for loiasis and lymph filariasis (combo with benzimidazole)
DEC contraindications
Onchocerciasis
DEC SE
Mazzoti like rxn
DEC mechanism and resistance
Mechanism:
Stimulate host immune response
Resist typically not a problem
Pyrantel indication (bug and stage)
- Nematodes in GI lumen
- larvae, adults
Pyrantel SE
GI, HA
Pyrantel mechanism and resistance
Mech:
- stimulate nicotinic receptor
- increase Ach release (spastic paralysis)
Resist typically not a problem
Praziquantel indications (bug and stage)
- trematodes (schistosomiasis)
- cestodes
- immature stages and adults
