antidepressants - TCAs Flashcards
MOA
block reuptake of serotonin and noradrenaline (different drugs to different extents)
clomipramine is more selective for which transmission
blocks reuptake of serotinergic
imirapmine is more selective for which transmission
noradrenergic transmission
TCAs can be roughly divided into those with ..
additional sedative properties
and those that are less sedating
Agitated and anxious patients tend to respond best to
sedatives
withdrawn and apathetic patients will often obtain most benefit from the
less sedating
TCAs with sedative properties
amitriptyline
clompramine
dosulepin
doxepin
mianserin
trazodone (not tca, serotonin-antagonist-and-reuptake-inhibitor)
trimipramine
the 3 TCAs with less sedative proprieties
imipramine
lofepramine
nortriptyline
This drug has a lower incidence of SE and its less dangerous in OD but is infrequently associated with hepatic toxicity
lofepramine
this drug has more marked antimuscarinic side effects than other TCAs
imipramine
these two are effective but particularly dangerous in overdosage, and one needs to be initiated by specialist
amitriptyline
dosulepin (specialist)
in the elderly, for initial treatment, use. ..
low doses
can you use TCAs for treating depression in children
no not effective
use of TCAs in elderly is potentially inappropriate STOPP criteria if used for
1st line antidepressant treatment - higher risk of adverse effects than with SSRIs or SNRIs
if prescribed in those with dementia, NAG, cardiac condition abmormality, Hx urinary retention (risk of worsening these)
