What are anti-depressants?

A

Medications to treat depression.
Off label uses include anxiety, pain, and insomnia.
(non-FDA approved for ADHD in ADULTS)

What are the most common types used?

A

Most common drugs used are SSRIs, ex: Fluoxetine, Citalopram, Sertraline, Paroxetine, and Escitalopram.

SNRIs may be used (vanlafaxine and duloxetine), and Buproprion (a ketoamine/NDRI drug also used in smoking cessation and SADD).

Older and less used are the tri and tetracyclics, as well as MAOIs.

How do patients respond?

A

Some people respond to some and not others, the cause of which is unknown.

Must ensure use and dosage are done correctly for at least 4-6 weeks to see effectiveness.

Always make sure to taper off medication so the body can adjust. There is NO addictive quality to these drugs.

Side effects of antidepressants.

A

Nausea, vomiting, weight gain, diarrhea sleepiness, and sexual dysfunction.

Combining SSRI or SNRI with a triptan medication (used in migraines) can cause serotonin syndrome. Use of MAOIs can also cause serotonin syndrome.

A

Zoloft, SSRI

Use in depression, OCD, panic disorders, PTSD, premenstrual dysphoric disorder, and social phobia in ADULTS.
Use in CHILDREN with OCD ages >6.

A

Prozac, SSRI

Use in depression, bulimia, MDD, OCD, panic disorder and premenstrual dysphoric disorder.
In CHILDREN: MDD in ages >8 and OCD >7.

A

Use is primarily OCD (in adults and children >8) and social phobia. But can also be used for depression, eating disorders, and chronic tension headaches.

Use in MDD, GAD, panic disorders, and social phobia. Sometimes used in OCD or fibromyalgia.

A

Remeron, NaSAA (tetracyclic antidepressant)

Use in MDD, anxiety, dysthymia, OCD and panic disorder. In low doses may be used for insomnia as an off label due to its hypnotic effects.

15-45mg PO HS dosing.

Buproprion

A

Wellbutrin, NDRI

Use in MDD, depression with seasonal changes, and smoking cessation. Can also be used for ADHD, bipolar disorders (depressive phase only), chronic fatigue syndrome, and lower back pain. Smoking cessation is also an indication.

150-300mg dosing.
