Heparin
A
Heparin prevents thrombin from converting to fibrinogen to fibrin.
IV or SQ
Protamine sulfate is the antidote
Avoid corticosteroids, salicylates, NSAIDs, green leafy vegetables and foods high in vitamin K
aPTT 20-36 should be 1.5-2 times (60-80)

Enoxaparin (lovenox)
A
Low-molecular weight heparin
Longer half life
Only SQ injection

Coumadin antagonizes vitamin K
A
Oral use
Continue IV heparin until therapeutic effects kick in
Don’t use w/low platelets
Mephyton (trade name for vitamin k) antidote
Doses usually BID
INR &PT 1.5-2 x higher
Don’t give to pregnant women/heparin instead
