Pseudomonas is a cause of HAP (in chronic chest infections pts) or UTI in immunosupressed people. What do you treat pseudomonas with?
Tazocin

Oral aciclovir
What do you use to treat C.Diff?
need an anti-anaerobic: metronidazole
How do you treat sepsis?
Sepsis - Amikacin (gram negatives) + Co-amoxiclav (G+ve)
How do you treat pharyngitis e.g. group A strep
Penicillin
How do you treat CURB-65 0-1
No hosp admission
use doxy or amoxicillin [also use in COPD H.influenzae]
if pregnant use erythromcin

Benzyl-penicillin and doxycycline
how do you treat HAP?
- Doxycycline
2. Levofloxacin
How do you treat aspiration pneumonia?
Cephalosporin (cefaclor) IV and metronidazole IV
how do you treat stomach bacteria?
norm anaerobes so METRONIDAZOLE
or IV benzylpenicillin
How do you treat GI and liver infections?
Send blood cultures and pus samples
- Co-amoxiclav and gentamicin
+ possibly metronidazole if suspect anaerobic
What is the treatment for uncomplicated UTI e.g. female and non pregnant + normal renal tract and function
nitrofurantoin for 3d if female or 7d if male
2nd line = co-amoxiclav
What is the treatment for complicated UTI e.g. male and pyelonephritis
Co-amoxiclav and amikacin [same as sepsis]
What is the treatment for a catheter UTI?
Only tx if symptomatic –> use gentamycin –> think: e. coli
What is the treatment for UTI if pregnant?
in 1st/2nd trimester use Nitrofurantoin (as trimethroprim as an antifolate)
in 3rd dont use nitrofurantoin use trimeth as NF causes haem of newborn
