Antibiotics Flashcards
Penicillin General
Pencillin G
- First gen
- treats: gram postive, enteroc and T. Pallidum ( syphillus)
Ox/ Diclox/nacfcillin
- second gen
- best against staph, only gram postive
Ampacillin and amoxicillin

- usually used for surgery
4.
Cefuroxime, Cefoxitin and Cefotetan
- 2nd gen and better - converage
- Covers HNPEK -Haemophilus, Nesseria, Proteus, ecoli and Klebsiella
AD
Disulfram rxn
Ceftazidime, Ceftriazone, cefotaxime
- 3rd gen more broud spectrum
- CAPES - Citrobacteria, Acinterbacteria, Psudomonas, Ecoli, Serratia
- Ceftazidime for psudomonas
Cefepime
- 4th gen
- used like cefazidime
- IV ONLY
Ceftaroline
- 5th gen
- broad + and - but cant treat psudomonas
- however can treat MRSA
Cephalosporins AD
- hypersensitve
- GI
- Alchol intolerance
- 1st gen nephrotoxic
Aztreonam
- MOnobactams and beta lactam sensitve
- only gram negative bc smol and can fit
- **no cross rxns ith penciliin or cephalosporins **
What med do we use to treat someone with a gram negative bacteria who is allergic to pencillin or cephalosporins
Axtreonam
Meropenem
- Carbapenems
- treats both + and -, anerobic and Psudomonas
Vancomycin
- Glycopetide - stops the crpss linking
- treat only gram positice and MRSA and Enteroccus
- 1st line for C. Diff but only oral
AD - Vancomycin flushing bc of histamine release
- Ototoxic w/ aminopsi
- nepho toxic
Daptomycin
- Lipopeptides - forms pores to kill bacteria
- only gram + and VRE
AD - BIG MUSCLE WEAKNESS
- CC with the statins
- monitor Creatine Levels
Gentamycin, tobramycin
- Aminoglycosides - crack membranes and bind with oxygen to get into the cell blocks 30s subunit
- Treatment only gram negative and pseudomonas
- can combo with cell wall inhibitors to attack gram positive
AD - Neph
- ototoxic
- high does muslce blocker
Linezolid
1.oxlianthione - blocks mrna from the 50s subunit
2.used only gram + bc efflux on gram negative kicks it out, MRSA, eneroc
3.VRE
AD
1. GI
2. Bone marrow
3. optic
4. CAN CAUSE SEOTONIN SYNDROME
Doxycycline and Tigecycline
- Tetracyclines binds to the 30s 2nd subunit of mRNA ribsome
- Docy is the 2 space 2
- Tigecycline is the god and hasnt been used a lot so can treat a lot of things except pseudomonas
- TIgecycline for VRE
AD
1.GI
2.GREY TEETH PURE CARE
3.Phototoxic
DDI
1. kids
2. pregnacny
3. Cations
Azithromycin
- Macrolides ( the crows) blocks the 50s of translocation
AD - GI
- Hepatoxic
- QT prolonged
DDI cyp interactions except Azithromycin
Clindamycin
- Blocks the 50s transloiation of rDNA
- mainly used in combo to provide coverage for anaerobes
- ONE OF THE BIG ANAEROBIC ABOVE DIAPHRAM
sulfoniamides combo with trimethprim has to (BACTRIM)
- inhibits folic acid synthesis
- can treat MRSA
- usually used for UTI
- HPEK treats
AD
1. GI
2. Hypersensitiviry rxn
3. hemologic effects
4. Leukopenia
5. hemolytic aniema (GD6P deficentcy)
6. kernitcus - frees bullirubin and gets into our basal ganglia in neonates
7. hyperkalemia
8. block cyp
9. protein binding
Contra
1. pregnacy
2. infants
Ciprofloxocin, Levofloxacin, moxifloxacin
- block super coiling glueing - just create strand breaks to cause cell death
- Cirpo can trea pseudomonas but not levo or moxi bc they made them more gram postitive
- made them more postice to use to treat respiroty antibitotics
AD
1. peripheral neropaty
2. photo toxic
3. seizures
4. MUSCOSKELETO - Tendonotos and tendon rupture
5. QT prolongation
CI
1. kids
2. pregnant
3. MAF Can give 2 befroe or 4 after
Metronidazole
- produg that generates free radicals that kill anaerobic bacteria
- USED FOR BELOW THE DIAPGHR ANEAROBIC
AD
1. GI
2. metalic taste
3. furry tounge
4. disulfam rxn
5. no alchohol
