  • peak serum plasma levels are checked 1 hour after administration and trough levels are checked before the next dose
  • if the trough level is high then the interval between doses needs to be increased
  • if the peak level is high then the dose needs to be decreased

A
  • time between the start of the infusion (x axis) is plotted against the plasma concentrations (y axis)
  • if the plot falls in the 24hr range then continue with regular dosing
  • if it falls in the 36hr range then dose every 36hrs
  • if it falls in the 48hr range then dose every 48hrs
