Antibiotics for CAP
CURB65 0-2 = amoxycillin PO or IV (doxycycline if allergic)
CURB 65 3-4 = IV co-amoxiclav and oral doxycycline (IV levofloxacin if allergic)
Antibiotics for CAP (CURB 65 <3)
Amoxicillin PO or IV
doxycycline if allergic
Antibiotics for CAP CURB65 >2
IV co-amoxiclav and oral doxycycline (IV levofloxacin if allergic)
Antibiotics for HAP
Non-severe - amoxicillin (doxy if allergic)
Severe - IV amoxicillin and gentamycin (co-trimoxazole and gent if allergic)
Antibiotics for aspiration pneumonia
Non-severe - amoxicillin and metronidazole (ORAL)
Severe - IV amoxicillin, gentamycin and metronidazole
Acute exacerbation of COPD management
Amoxicillin (doxy if allergic) if signs of bacterial infection
Antibiotics for epiglottitis
Ceftriaxone IV
Antibiotics for tonsillitis
Penicillin V (oral) if scores > 4 on FEVERPAIN score
Otitis media management
avoid Abx if possible. Amoxicillin in <2 years and bilateral or perforated eardrum
Otitis externa management
Acetic acid if mild
Topical abx and consult ENT if moderate/severe
Oral candida management
miconazole gel or nystatin
Bacterial Endocarditis management
IV flucloxacillin if severe
IV amoxicillin if indolent
How is bacterial endocarditis diagnosed?
3 sets of blood cultures taken 6 hours apart if possible
At least 2 sets 1 hour apart if septic
