Discitis/ Vertebral Osteomyelitis

  • vancomycin PLUS
    • cefepime / ciprofloxacin

Requires culture of bone/blood/disc to ID cause. can withold empiric Abx to definitive treatment. High risk pt = IV drug use, endocarditis, neurologic deficits.

    2. PCN allergy: vancomycin
  2. Type III
    1. gentamicin PLUS one abx from step 1.

pip/tazo if concerned for clostridia from soil contamination.

Type I: open fracture with < 1 cm clean lac.

Type II: Open Fx > 1cm lac.

Type III: open Fx with extensive soft tissue damage
