Metoprolol
5 mg IV q5min (max 3 doses)
Diltiazem
0.25 mg/kg
Amiodarone: Pulseless Vtach, Vfib, Vtach with pulse, SVT
Pulseless Vtach/Vfib: 300 mg IV bolus with 29 cc 5%dextrose flush followed by 150 mg bolus IV PRN q5min
Vtach with pulse/SVT: 150 mg in 100 mL over 10 min followed by 1 mg/min drip over 6 hours followed by 0.5 mg/min for 18 hours
Cardioversion Joules
Afib/flutter with RVR: 200 J
SVT: 50-100 J
VT with pulse: 100 J
Atropine
0.5 mg q3-5 mins (max 3 mg)
Epinephrine for pulseless
1 mg (1:10,000) IV q3-5 min 0.01mg/kg IV (0.1 cc/kg of 1:10,000)
Epinephrine drip for bradycardia
Initial 2-10 mcg/min of 1 mg in 500 mL escalating q3-5 min
Defibrillation Joules in adults
Vfib/VT without a pulse: Defibrillate 200J for adults
Adenosine in adults
6 mg –> 12 mg in adults
Adenosine in kids
0.1mg/kg –> 0.2 mg/kg in kids
Defibrillation Joules in kids
2 J/kg –> 4 J/kg for kids
Cardioversion Joules in kids
Kids: 0.5 J/kg
Amiodarone in pulseless Vtach/Vfib in kids
5 mg/kg bolus
Atropine in kids
0.02 mg/kg (min 0.1), may repeat once
Magnesium
1-2 g in 10 ml 5% dextrose in water over 1 min
