what is it?
autoimmune disorder
how does it manifest?
recurrent venous or arterial thrombosis and/or foetal loss
what are causes?
HLA-DR7 + environmental triggers
primary = young women
secondary = association with SLE
what is pathophysiology?
antiphospholipid antibodies react against phospholipids on plasma membranes
what does the presence of anti-phospholipid antibodies lead to?
hypercoaguable state
how does is clinically present - venous?
DVT/PE
livedo reticularis
what is livedo reticularis?
blood clots in capillaries lead to swelling of venules resulting in purplish, net-like discolouration of skin
how does it clinically present - arterial?
increased frequency of stroke or MI
what investigations are used?
bloods
what will bloods show?
thrombocytopenia
prolongation of aPTT
how is it diagnosed?
1 clinical + 1 lab diagnosis
what antibodies will be positive?
antiphospholipid antibody
lupus anticoagulant
anti cardiolipin
anti-beta 2 glycoprotein
what is management?
lifelong anti-coagulant
what is 1st line anticoagulant?
LMWH then warfarin or aspirin prophylaxis
what is the management for pregnancy?
LMWH and aspirin
