Anti-arrhythmics Flashcards
Anti-arrhythmic MOA
Correct or prevent arrhythmia
Class I
Na+ channel blockade
Class II
Beta-blockade (Non-DHP’s)
Class III
Prolonged repolarization (Amiodarone)
- high risk of causing torsade (TDP)
Class IV
Ca2+ channel blockade (Non-DHP’s)
TDP or torsade
Life threatening heart rhythm disturbance, ventricles out of rhythm.
Amiodarone
Lower risk of TDP then other antiarrythmics
acts as all classes but technically it is class III
Amiodarone side affects
bradycardia, dermatologic toxicity, drug interactions, pulmonary fibrosis, pneumonitis, cardiotoxicity, liver toxicity, ophthalmic toxicity
Half-life = 25-100 days
Rule of 5
Takes 5 half-lives of drug to be fully out of your body
Steady state of a drug
Moment drug therapy where the amount that is being put in the body with every dose is perfectly replacing the amount that is taken b/w doses
Flecainide class
1C
Flecainide use
A-fib (patients without structural heart disease)
Flecainide side affects
dizziness, visual disturbances, dyspnea, QT prolongation/TDP
Flecainide and TDP
Low risk of TDP in patients without structural heart disease)
Which drug may require the use of a loading dose
Amiodarone
