What is HELLP syndrome?
H - (haemolysis) the breakdown of red blood cells
EL - (elevated liver enzymes) a high number of enzymes in the liver is a sign of liver damage.
LP - low platelet count
A rare liver and blood clotting disorder
Causes of HELLP syndrome?
Cause unknown
Complication of pre eclampsia or eclampsia
Preeclampsia or eclampsia in previous pregnancy
Family history of pre eclampsia or HELLP
Autoimmune diseases
Multiparity
Covid during pregnancy
How do you help someone struggling with their mental health who already has a PN mental health referral?
Make sure they feel safe at home, make sure feel like they’re not going to act on their thoughts or feelings, if they do feel like they’re going to harm themselves in anyway make sure they ring 911/ or mental health crisis line, take self to A&E or triage.
What do you do if someone had bacteria in there urine previously but didn’t receive antibiotics, no longer has UTI symptoms?
explain symptoms of UTI
Make sure definitely has no symptoms of UTI at present.
Send off one that day.
To bring us in a urine sample to send off if feels like has a UTI occurring after the point of receiving antibiotics.
Make sure keep hydrated
Indications for GTT
At 34 wks
Ethnicity (black Afro Caribbean background)
Previous GDM
First degree family history of diabetes
Baby measuring big
Glucose in the urine
PCOS diagnosed with a blood test
What additional things would you ask at 28wks?
Any family history of VTE?
Any varicose veins?
Any swelling in legs?
Blurred vision, shoulder tip pain?
Update mental health questions
Update lifestyle questions
Ask DV question
Check they’ve had GTT if indicated at 24 wks if indicated, or if need one booked.
Check if need anti D booked and make sure they’re booked in for a group and save 5 days before anti D appointment.
Check they’ve got there MATB1 form.
What additional things would you do at a 16wk appointment
Promote antenatal classes and vaccinations
Listen in to FHR
