what is it?
chronic inflammatory disease of axial skeleton (vertebral joints)
what can it lead to?
partial or complete fusion and rigidity of spine
what is pathophysiology?
becomes more kyphotic over age - until disc is fused
what are clinical symptoms?
“A” disease
anterior uveitis
aortic insufficiencies
apical pulmonary fibrosis
what are the articular symptoms?
spinal and neck pain - gradual onset of dull pain that progresses slowly
morning stiffnses >30 mins improves with activity
question mark spine - late
what causes question mark spine?
loss of lumbar kyphosis and pronounced cervical lordosis
what are clinical signs?
schobers test - 5cm below, 10cm above, should be >20cm, measures lumbar spine flexion
reduced chest expansion
iliac crest - pain on palpation (inflammatory)
what investigations can be used?
bloods
x-ray
MRI
what is 1st line test?
x-ray
what will bloods show?
raised inflammatory markers
HLA B27
what will x-ray show?
BAMBOO SPINE
may show sclerosis and fusion of sacroiliac joints
what is bamboo spine?
ossification of annulus fibrosis (lumbar spine)
what will MRI detect?
sacroiliitis and earlier changes e.g. bone marrow oedema and enthesitis of spinal ligaments
what is 1st line management?
NSAIDs
what is treatment?
same as spondyloarthropathies
