So what makes the lower back pain specific to AS?
More than 3 months, morning stiffness, before 40years old
Physical AS findings?
Restricted spine movement, bad posture, sacroillitis
Commonest places to find Enthesitis?
Achilles and Costochondral regions
Patients are radiographed for their sacroillitis, that’s then graded based on the New York grading system, what’s that?
Grade 1 suspicious, grade 4 total ankylosis
What do the X rays show?
Bamboo spine and shiny corners
How do we best visualize Enthesitis?
MRI or US
How to diagnose?
Sacroiliitis, back pain >3months, limited lumbar mobility
Goal of Treatment
No cure, only supportive
Treatmemt regimen
NSAIDS and corticosteroids
When should we do surgery?
Hip replacement
