Ankle Flashcards
lecture & lab
ankle
When would you perform an ankle ligament stress test?
- when you need to rule in/rule out a diagnosis
- when you are concerned about a certain MOI
- subjective complaint
- observation (walking/body analysis/ swelling)
- population (does their symptoms fit w diagnosis/tests)
- location of their pain (aligns with other signs)
- duration of symptoms (chronic/acute, specifc/nonspecific)
ankle
When would you perform a distal tibiofibular joint mobility assessment?
- when you are wanting to rule in/out daignosis
- when you see location of pain + MOI
ankle
when and why would you perform a body region screen?
ankle
describe the movement and direction of force when performing a dital tib-fib joint mobility assessment
the heel of the clinicians assessing hand holds the anterior surface of the distal fibula. using their assessing hand, the clinican glides the fibula in a posteriro direction along the plant of the joint.
ankle
what are some reasons you would assess mobility at the distal tib-fib joint?
what does reproduction of symptoms indicate when performing?
done for joint play/passive accessory motion, client response and end feel.
reproduction of concordant pain suggests dysfunction. impaired joint mobility or end feel may also suggest dysfunction if pain is also reproduced.
ankle
distal tib-fib joint mobility
primary:
secondary (note, not a true positive finding)
primary: joint play (accessory motion), end feel
secondary: reproduction of symptoms
ankle
what is the SN, SP and +LR, -LR of the talar tilt test?
lateral vs medial?
lateral:
SN: 67
SP: 75
+LR: 2.7 -LR:0.44
medial:
not reported
ankle
what are the primary ankle sprain grades?
secondary?
grade 0: no laxity
grade 1: mild laxity
grade 2: moderate laxity
grade 3: gross laxity
secondary:
reproduction of symptoms
ankle
what does the PTFL resist?
posterior movement of the talus
ankle
what does the ATFL resist?
torsion and inversion stsresses in plantar flexed foot
ankle
what does the CFL resist?
torsion and inversion in dorsiflexed foot
with an ankle ligament stress test such as the anterior drawer test/revserse anterior drawer test what would indicate a + test?
1 cm of movement compared to uninvolved side
remember R1 and R2 when performing
What ligament is being stressed with the lateral (eversion) talar tilt test?
deltoid ligament - stressing the medial side of ankle
when performing the talar tilt test what postion would stress the CFL more?
CFL = calcaniofibular ligament
a more vertical postion
when performing the talar tilt test what position would stress the ATFL more?
10° of plantarflexion
think about end feel when performing
when performing the talar tilt test what position would stress the PTFL more?
thinking of breaking the bread
increase DF + twisting calcaneus
stand on ipsilateral side when performing
think about end feeling when performing
resisted ankle inversion/eversion
perfom? why?
this is not a true MMT - we are assessing movement quality and reproduction of symptoms …is it weak? strong? painful?
performing - hand placement is everything they will engage different muscles according to where your hand is. ensure you are doing what you think you are doing.
inversion = pressure is in and up
