Stepwise Mx of angina?

A
  1. Rate-limiting Ca channel blocker/B-blocker
  2. Ca channel blocker (dihydropyridine) + B-blocker
  3. Add ranolazine/nicorandil/ivabradine/isosorbide mononitrate
  4. PCI

Titrate medication to maximum dose tolerated before going onto next Tx plan
