1st Generation cephalosporin
least spectrum of coverage. Mostly Gram + (cefazolin)
2nd Generation cephalosporin
Have more gram - coverage. (cefoxitin, cefurxoime, cefaclor)
3rd generation cephalosporin
ceftriaxone and ceptizoxime
4th generation cephalosporin
Broadest spectrum of coverage. Gram - and gram +. (Maxiprime ane Cefelidine)
Bactericidal
Activly kill microrganisms
cephalosporins, aminglycosides, fluoroquinolones, vancomycin, daptomycin, metronidazole
Bacteriostatic
Slow groth of microorganisms
macrolides, tetracyclines, trimethoprim, sulfonamides
Cephalosporin
B-lactum ring inhibits cell wall synthesis
Clean procedures
Cefalozin 1-2 g
Thoracic / orthopedic
Ceuroxime 1.5g
Bowel
Cefoxitan 1-2g or Cefazolin 1-2 g + metronidazole 500mg, Ampicillin / sublactam 3g
PCN allergies
Vancomycin 1g or Clindamycin 600-900mg
Gram - coverage may be added w gentamycin, cipro, levo and aztreonam
Vancomycin
inhibits cell wall synthesis. Most gram + 15mg/kg. prevention of MRSA, prostetic heart valve and vascular graft. Caution VRE. Redose 12hrs.
Neurosurgey
Cefazolin 1-2g. Nafcillin 1-2g. Vanco 1g.
Head and Neck
Cefazolin 1-3g or Clindamycin 600-900mg + gent 1.5mg/kg
Cardiac Surgery
Cefuoxime. Vanc and clin are alternatives.
