Q

what are the three longitudinal (paired) muscles of the pharynx?

what innervates them?

A
  1. stylopharyngeus = CN 9
  2. palatopharymgeus = CN10
  3. salphingopharyngeus = CN10

Q

what 3 things travel through the “gaps to the mouth” between the constrictor muscles?

A
  1. stylopharyngeus longitunidual pharynx muscle
    • its innervation, CN9
  2. also the lingual artery which supplies the oral floor + the tongue

14
Q

where does the salpingopharyngeus muscle originate?

A

near the eustachian tube opening in the nasopharynx - starts at the salph. fold which is part of the curvy bit

16
Q

what veins do the deep cervical + superficial cervical lymph nodes lie on?

A
deep = internal jugular vein
superficial = external jugular vein
17
Q

name the 4 tonsils from superior to inferior.

A
  1. pharyngeal (AKA adenoid )
  2. tubal
  3. palatine
  4. lingual tonsil
18
Q

what spinal levels is the larynx at?

A

C 4-6

19
Q

functions of the larynx

A

URT patent
Stops aspiration into LRT
Sounds - vocal cords
Cough reflex involvement

20
Q

what is “cricoid pressure”

A

a technique used in endotracheal intubation. pressing down on the cricoid cartilage onto C6 to obstruct the oesophagus while leaving the airway open.

it lowers risk of aspiration in the procedure

aka Sellick maneouvre

21
Q

what is the aryepiglottic fold?

A

the superior border of the quadrangular membrane…

which is between the Vestibular ligament (false VC) and the epiglottis.

22
Q

where is the conus elasticus?

A

between the vocal cords + the cricoid cartilage

23
Q

what is the name for the depression just behind the root of the tongue between the folds in the throat.

A

Vallecula

where the laryngoscope is put (between epi + tongue)

24
Q

innervation of the intrinsic larynx muscles?

A

CN 10 for all of them

  • cricothyroid is external laryngeal nerve
  • all others are internal laryngeal nerve
25
Q

when does inferior laryngeal nerve begin

A

at the cricothyroid level (from left recurrent laryngeal nerve)

26
Q

what muscle is used for forced respiration + loud speaking?

A

abductor = Posterior cricoaryntenoid

27
Q

what muscle is used for normal breathing + low pitch?

A

relaxors = thyroaryntenoid

28
Q

what muscle is used for high pitch?

A

tensors = cricothyroid

29
Q

what muscles contract to allow whispering?

A

adductors = specifically the lateral crico-aryntenoid

30
Q

a tumour starts growing just above the hyoid level. what LNs will it spread to?

A

supra glottic = superior deep cervical lymph nodes

31
Q

a tumour starts growing on the thyroid cartilage. what LNs will it spread to?

A

subglottic = paratracheal nodes. affect voice + airway

32
Q

a tumour starts growing on the epiglottis. where will it spread?

A

95% wont - just stay there. will affect voice + airway though

33
Q

explain how sound is made

A
  1. controlled expiration helps build subglottal pressure
  2. SG pressure threshold reached
  3. air across the cords = vibrations
  4. the vibration buzz is amplified by the oral/ nasal cavities + pharynx
34
Q

explain how sound can come out of the nose instead of mouth - what nerves are involved?

A

v3 = tenses the soft palate

CN 10 = descend the soft palate to close the oropharynx

CN12 alters tongue shape
CN 7 alters teeth + lips

35
Q

explain how sound can come out of the mouth instead of the nose - what nerves are involved?

A

V3 = tense the soft palate
CN 10 = elevate the soft palate to close the nasopharynx
CN 12 alters the tongue shape
CN7 alters teeth + lips

36
Q

the muscosa of the larynx is supplied by what 2 nerves?

where do they join?

A

above vocal folds = internal laryngeal
below = inferior laryngeal
join @ GALEN’S anastomosis at vocal folds

37
Q

CN10 innervates the palate + pharynx + larynx. how do you test that it is working in each site?

A
palate = SAY AHHHHH (moves away)
pharynx = sip some water - splutter?
larynx = are they hoarse?

also ask em to cough to test vagal control of diaphagm etc
