What is anaphylaxis?
Serious allergic reaction that is rapid in onset and may cause death
Who is at risk of anaphylaxis?
Acute onset of an illness
Symptoms involving 2 or more organ systems
Reduced BP following exposure to a known allergen
What are the most frequent signs and symptoms of anaphylaxis ?
Urticaria/angioedema- most common
Upper airway edema
Dyspnea/wheeze
Flush
What drug does not block anaphylaxis ?
Diphenhydramine
What are some triggers of anaphylaxis ?
Food- most common
Drugs
Insects
Who are in the high risk group for latex allergy?
Rubber industry workers
Healthcare workers
Children with spina bifid
Children with multiple surgeries
What are the products containing latex that cause common reactions ?
Gloves especially powdered
Balloons
Condoms
Dental dams
What are some food allergies that are correlated to latex allergy?
Avocado Banana Celery Chestnut Kiwi Melon Passion fruit
What are the different phases of anaphylaxis ?
Uniphasic
Biphasic- symptoms recur up to 8 hours later without further exposure to the trigger
Protracted-worse one: symptoms worsened despite treatment
How to diagnose anaphylaxis?
Complete and accurate medical/allergy history
Skin test/ specific IgE tests
Serum tryptase measured within 3 hours
What is the treatment for anaphylaxis?
When in doubt inject epinephrine
Speed is critical
Assess airway, breathing, circulation
What is the dose of epinephrine to use?
1:1000 dilution, 0.3 mL for adults
0.01/mg/kg in children
Many patients require more than a single injection
What are some secondary measures to take in case of anaphylaxis ?
Place patient in recumbent position and elevate legs
Maintain airway
Oxygen, 6-8 liters/minute
Normal saline IV
