Keine signifikante Unterschiede in Lokalrezidive, Fernmetastasen, Kolostomie. Es zeigte sich ein Trend, dass die Induktionschemo ein bisschen schlechter ist. Das hat sich tatsächlich auch im DFS gezeigt, dass das Krankheitsfreie Überleben im Induktionsarm signifikant schlechter war.
Eine Induktionschemotherapie ist vielleicht strahlenbiologisch nicht sinnvoll, weil es zu einer Selektion von resistenten Klone führt.

Neither of the two strategies investigated—chemoradiation with cisplatin, and further maintenance chemotherapy using fluorouracil and cisplatin—is more effective than standard care with mitomycin for achieving complete response, progression-free survival, or overall survival. Our results show that fluorouracil and mitomycin and 50·4 Gy radiotherapy in 28 daily fractions should be standard practice.

Gemeinsam mit Proktologe. Hier ist es wichtig, dass man gerade bei größeren Tumoren nicht so früh auf die Salvage- Weg geht. Hier gibt es eine schöne Arbeit von ACT II (Glynn-Jones et al., Lancet Oncol 2017). Es war vorgeplant, dass man aus verschiedenen Zeitpunkte ein Response-Assessment nach RCTx macht. Bildgebung war zu 26. Woche obligat.
