Acute adrenal insufficiency
Corticosteroid + anaesthetics = severe hypotension
Stop before surgery
Combined contraceptives → VTE - Stop 4 weeks before major surgery, leg surgery or prolonged immobility
HRT → VTE
- Stop 4-6 weeks before surgery + restart on full mobility
Antidepressants
- MAOI: hypertensive crises
- Stop 2 weeks before surgery
- TCA: arrhythmias + hypotension
Lithium → constant fluid + electrolyte balance
- Stop 24 hours before major surgery
Oral anticoagulant → bleed
- High VTE risk: bridge w heparin
Diabetes + surgery → hyperglycaemia
- Minor surgery: stop metformin, sulphonylureas + SGLT2
- Major surgery: sliding scale insulin
Potassium sparing diuretics → hyperkalaemia
ACEi/ARB → hypotension
Do not stop
- Anti-epileptics
- Anti-parkinsonian
- Anti-psychotics
- Anxiolytics
- Bronchodilators
- CV drugs
- Glaucoma drug
- Immunosuppressants
- Progestogen only contraceptives
- Thyroid + antithyroid drugs
