AMK teach Flashcards
Stevens–Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are now believed to be variants of the same condition, distinct from erythema multiforme. SJS/TEN is a rare, acute, serious, and potentially fatal skin reaction in which there are sheet-like skin and mucosal loss.
SJS - just spots
TEN is over chest heavy
Toxic epidermal necrolysis TEN covers more 30 of the body surface area what are the most common causes of this
onset is few days or months
systemic toxicity and prevention
histology is full thickness epidermal necrosis with sub epidermal detachment
management of this is stop cessation of causative drug , ITU fluids, temp control, pain relief and eye and skin care
antibiotics - penicillin and sulphonamides
anti epileptics e.g. phenytoin and carbomazopine
NSAIDs
prodrome rash and illness then macular rsh and painful ulcers and blisters
pyrexia dn tacky carina
necrotising fascitis looks like a whole - rapid spreading infection of deep fascia with tissue necrosis. Purpura and blistering
common in health patents after trauma
what organism causes this
haemolytic group A strep bacteria
debridement and IV abs
eczema herpticum - derm emergency around the mouth and dematome - viral infection casing fever and clusters of itchy posters , atopic enzcema seen in children.
what is the cause and treatment
herpes simplex type 1 ( orofacial and infant ) type 2 is genitals and adults
PO acyclovir 10-14days and 400-800mg , if sick IV

if affects body reign then topical antifugnal oral if hair site
foot ringworm is athletes for and topical anti fungals wg clotrimamzole
fungal infection of nail from nail clippings to be diagnosed what is it
onchomycosis
impetigo much like eczema herpiticum howler most common in children with superficial golden crusted skin lesion around mouth and very contagious and can be complication of eczema
what is the cause
staph aureus
oral flucloxacillin 7 days
off school until crusts dried out
complications is scalded skin syndrome SCCC
SCCC-staph scaled skin syndrome - red blistering skin which looks like a burn or scald. normally adults with renal failure or neonates or under 5 and treatmedn is flucoxacillin what is the presentation
widespread redness like burn on skin skin comes off on sheets fever irritability fluid filled blisters then burst
intraepidermal blistering
SCCC-staph scaled skin syndrome - red blistering skin which looks like a burn or scald. normally adults with renal failure or neonates or under 5 and treatment is flucoxacillin what is the presentation
widespread redness like burn on skin skin comes off on sheets fever irritability fluid filled blisters then burst
intraepidermal blistering
Bullous disorder are both autoimmune
Bullous pemphigoiD - elderly, sub epidermal split , blisters are Deep and fluid filled and itchy
less involvement of mucosa affecting trunk and limbs
MX is oral steroids
pemphigus vulgaris is the younger 30-60
intra epidermal split/bullae
more superfiicla
flaccid ruptured blisters and painful
mucosal involvenetn usually moth then skin high mortality rate high dose oral steroids and immunosuppression
spot diagnosis, nodular lesion , most common on shin
erythema nodosum
what is the result of repetitive scratching lichen simplex chronicus
area of thickening skin from repetitive scratching
erythema migrant rash - bulls eyes found on what disease
Lyme disease
what organism causes lymes disease
borrelia burgdoferi
dermatitis herpetiformis is a blistering skin manifestation of which disease
coeliac disease
