What is the main mechanism of action of Amiodorone?

A

Blocking potassium channels which inhibit repolarisation and hence prolong the action potential

  • can also block sodium channels

Which factors limit the use of amiodorone?

A
  1. Very long half life (20-100 days)
  2. Pro arrhythmic effects due to lengthening of QT interval
  3. Ideally given in central veins - causes thrombophlebitis

What are the main adverse organ impacts of taking amiodorone?

A
  1. Thyroid dysfunction - hypothyroidism and hyperthyroidism
  2. Lungs dysfunction - pulmonary fibrosis / pneumonitis
  3. Liver fibrosis - liver fibrosis / hepatitis
  4. Heart - bradycardia / prolonged QT interval
