Meds used to tx Alzheimer’s
ACH inhibitors = increase ACH
1) donepezil ( aricept )
2) rivastigmine ( exelon ) - comes in a patch
3) galantamine ( razadyne)
NMDA receptor blocker
4) memantine ( Namenda )
Combo of aricept and NMDA blocker
5) ER memantine + donepezil ( namzaric )
Donepezil
Aricept
Indication: mild to moderate to moderate to severe ( only one indicated for both. Rest of the ach inhibitors only for mild to moderate )
Comes ODT ( if elderly patient cant swallow this is helpful )
Rivastigmine
Exelon
Comes in patch ( benefit once daily instead of bid and less nausea with patch )
Caps 1.5 mg bid with food ( nausea )
Indication: mild to moderate Alzheimer’s
Galantamine
Razadyne
Indication: mild to moderate Alzheimer’s
Watch for serious skin reactions. D/C if skin rash occurs
With food ( GI upset )
Renal and hepatic impairment adjustment needed
Cholinesterase inhibitors Side effects
increases ACH: SLUDGE
Bethanechol
Urecholine : urinary retention
Reversal toxic anticholinergic effects antidote
Physostigmine
Tx of acute toxicity of cholinergic agent ( acetylchoinesterase poisoning)
Atropine
Increase heart rate for bradycardia
GI disorders
Respiration
Inhibiting salivation and secretions
0.5 - 1 mg IV q 3-5 mins max 0.04mg/kg
Memantine
Namenda
NMDA receptor antagonists: blocks NMDA R and this decreases glutamate in the CNS and therefore decreases the damage in the CNS
Donepezil + memantine
Namzaric
ER capsules
If you need to open capsule and sprinkle on applesauce
