ทำไมเกิด autoimmune จาก infection
- molecular mimicry : Ab ไปจับ self-Ag ที่คล้ายกับเชื้อโรค e.g. M protein of GAS เกิด cross-reac w heart muscle lead to rheumatic heart disease
- polyclonal activation : เชื้อโรคบางตัว e.g. superantigen สามารถกระตุ้น T-cell ได้เยอะเกินน ยั้วเยี้ยะ ก้เลยมีโอกาสเกิด autoreactive T-cell ได้ (T-cell ที่ทำงานมั่วซั่ว)
- homeostasis disturbance : lead to aberrant immune regulation e.g. ในรูปที่อจแปะมาคือ cytokine เสียสมดุล งง เลยไป attack self tissue
- release of sequestered Ag by injury at immunoprivilege site พวกตา สมอง : คือปกติโซนนั้นจะไม่มี immune response แต่พอเกิด injury อาจมีไปเจอกับพวก immune cell ได้ ส่งผลให้เกิด immune response แบบ systemic .. แน่นอนว่าไปยังตาข้างที่ปกติด้วย เลย injure BOTH เลย ;-;

- 1st degree : epith, no blister, sunburn, a bit painful, 2-5 วัน, no scar but may discolor
- 2nd degree : epith to top dermis, moist & blister, flash burn or chemical, 5-21 or 21-35 วัน, minimal to no scar but may discolor
- 3rd degree : epith and dermis, leathery dry no elastic, flame hot surface electric, no pain, เล้ก ๆ เดือนนึง ใหญ่ก็ grafting, have scar !
** 2nd deep to 3rd จะเจอ hypertrophic scar and contracture ฟีลผิวดึงรั้ง

- รวมโรคที่ positive 100% : SLE, drug-induce lupus, MCTD (mix CNT disease)
anti-native DNA Ab (DS-DNA)
- substrate Ag : kinetoplast of crithidia luciliae
- Ab from pt’s serum bind to specific Ag
- highly specific for SLE

- p-ANCA : Ab to MPO, non-specific เจอได้ใน vasculitis SLE PAN
- c-ANCA : Ab to serine protease in azurophilic granule, 90% positive in wegener’s granulomatosis

- fever, stinging eye, pain upon swallow, early mucosal involve 2+ site, hemorrhage erosion of eye mouth genital
- dusky red, atypical target lesion, flaccid blister - nikolsky sign positive
- pathogenesis : activity of drug-specific cytotoxic T cells ↑ → release of granulysin (a cytolytic protein) by an unknown mechanism → damage to keratinocytes ทำให้ cell lysis รัว ๆ

- active border แดงคัน อาจเจอ incognito, majocchi ได้บ้าง // ตอนตรวจขูดแค่ขอบ
- TEMN trichophyton epidermophyton microsporum nannizia : keratinophillic fungi ขนผมเล้บบ
- ติดจาก คนคน สัตว์คน สัตว์สัตว์ มันเลยดิ
- branching septate hyphae with arthroconidia (แก่แล้วหักเป็นปล้อง)
- dermatophytid reaction : inflam reaction ที่ขึ้นห่างจาก primary infection (ติดราจุดนึง ผื่นขึ้นอีกจุด)
- gold standard for dx คือ fungus culture
- tinea capitis ต้องใช้ยากิน, tinea unguium เป็นเยอะต้องกิน
candidiasis
- satellite lesion, well-demarcated // ตอนตรวจขูดทั่วรอยผื่น
- เชื้อ candida - budding yeast and pseudohyphae
- but เอาเชื้อไปย้อมแกรมจะเจอ snowman 5555555555
- tx ก้คล้ายอันอื่น but pls keep skin dry, diaper area may use ointment or cream
KOH
ตรวจราก้มีแต่ไอนี่แหละ บล้อกนี้น่ะนะ
10% KOH for skin specimen, 25% for nail specimen
KOH จะ digest keratinous material ทำให้เห็นราชัดขึ้น
incubate in room temp 5-30 min or passing over fire
** อย่าเอาราไปแช่ตู้เย็น เดี๋ยวตาย
Chromoblastomycosis / chromomycosis
- rash : verrucous เป็นกะหล่ำ แห้ง hyperkeratotic w black dot
- histo found dark brown round septate bodies , chromo body , muriform
- เชื้อกลุ่ม dematiaceous fungi / melanized fungi e.g. phialo fon rhino clado
- chronic infect or trauma อาจทำให้อาการหนักลามไป subcu
Mycetoma
- triad : mass (tumefaction), sinus drainage, grains ในแผลมีหนอง ในหนองมี grain (คล้ายเส้นขนกระจุก)

1. Eumycotic mycetoma ดำ : เกิดจาก dematiaceous fungi, hyaline fungi e.g. madurella exo scedo
