- Hx of Heparin induced Thrombocytopaenia (HIT)
- Severe hepatic impairment / disease, including oesophageal varices
- Recent trauma or surgery (<3 weeks)

- Significant head injury in past 3 months
- Major trauma in past 3 months
- Stroke / TIA in past 3 months
- ICH at any time
- GI/GU bleed in past month
- Current bleeding disorder, active bleeding (excluding menses) or bleeding tendencies
- Anticoagulants or glycoprotein IIB / IIA inhibitors
- Allergy to tenecteplase or gentamicin
