Symptoms of child with malignant disease
- fatigue, malaise, fever (general symptoms)
- look for these general symptoms over long periods of time
What kind of pain is characteristic for malignancy
- wakes child up at night
- localized
- bone/leg pain (ALL)
First thing to do with a child with ALL
CBC to look for leukocytosis.
If all 3 cell lines enhanced is suggestive.
What is the second step for ALL
Look for LDH, uric acid or higher potassium (from cell damage)
Look for symptoms for ALL
Enlarged lymph nodes, hepatosplenomegaly, petechia, and pallor from anemia
After symptoms do a what to look for ALL
Bone marrow biopsy and blood smear (looking for lymphoblast)
Where is the bone marrow biopsy taken from?
Posterior iliac crest
What comes after the bone marrow biopsy
Genetic testing. Put in group (low, intermediate, high risk) determines amount of chemo later
How do you check CNS for ALL
Lumbar puncture and follow up if there is need steroids and intrathecal chemotherapy
First line therapy in leukemia last for
1 year
How is the chemo administered in children
Not an IV needle (not peripheral since it isn’t safe). Give a central line (Porto-cath catheter).
