Alcohol Flashcards
Alcohol abuse
Social life impaired for at least 1 mo
Alcoholism
Tolerance and dependence from prolonged alcohol abuse
Different response to alcohol in people more prone to become alcoholics
Marked increased in release of B-endorphins in dopamine reward pathway
Alcohol absorbed in
Stomach and small intestine
Alcohol metabolized by
ADH, MEOS/CYP2E1
Alcohol metabolism kinetic property
Zero order kinetics (rate independent of concentration)
Cofactor required for oxidation by ADH and ALDH
NAD+
Saturated NAD+ at low concentration causes
Less NAD+ for other E –> lactate and acetyl-CoA accumulation –> FA synthesis + lactic acidosis
Chronic alcohol induces which metabolic enzymes
CYP2E1 (2E1) and MEOS (CYP450s)
ASIANS LIKE ME LACK
ALDH
WOMAN LIKE ME HAVE
Lower ADH
Drug inhibits ALDH
Disulfiram
Drug inhibit ADH
Fomepizole
Alcohol effects GABA and NMDA receptors by
Down-regulating GABA receptors, up-regulate NMDA receptors
Cross tolerance of alcohol to
Benzo, Barbiturates
MOA of Alcohol
Binds GABA(a) receptor --> increase Cl- influx --> enhance inhibitory GABA transmission Increase DA in mesolimibc pathway
Anterograde amensia due to
blockade of NMDA receptors
CNS effect with low concentrations of alcohol
disinhibition, decreased anxiety, mild euphoria, confidence increases, memory, concentration, mood swing
CNS effect with high concentration of alcohol
motor fxn and judgement are impaired, speech slurs, and ataxia
Effects of alcohol (immediate)
CNS effects
Smooth muscle: Vasodilator –> hypothermia
Heart: Depression of myocardial contractility
Kidney: Diuretic
Tx of acute alcohol toxicity
Mgmt of resp depression, prevent emesis
Hangovers due to
buildup of acetaldehyde, dehydration, and beginning of withdrawal
If you have seizures from acute alcohol toxicity, treat w/
Lorazepam, phenytoin
Effects of chronic abuse of alcohol on your liver
Gastritis, pancreatitis, liver dz, alcoholic hepatitis, liver cancer
