What are the 3 main components of assessing an airway?
Look, Listen, Feel
What are you assessing when looking?
Are they patent?
Are they partially or completely obstructed ?
Visible obstructions?
Is there any visible chest movement? (this does not ensure patency)
What are you assessing when listening?
Are they audibly breathing?
Is the child able to talk/cry (demonstrating patency) or are they making any airway noises?
What are the 3 airway noises?
Stridor, gurgling, snoring/stertorous
What are you assessing when feeling?
Can you feel any breath on your face?
Or can you see any misting on the O2 mask?
How long are you assessing look, listen, feel for?
10 seconds per assessment
When would you use an airway opening manoeuvre ?
If reduced level of consciousness
What are the airway manoeuvres?
Head tilt/chin lift
Jaw thrust
What is the intervention for snoring?
Reposition airway, use airway opening manoeuvres- chin lift into neutral (baby) or sniffing position
Use a jaw thrust (if c-spine injury)
What is the intervention for gurgling?
Suction with a soft tip catheter at 12-20kPa via wall suction
What are the interventions for stridor?
Seek immediate senior assistance- do not attempt to suction as not to upset child. Allow them to position as comfortable
What is the consequence of a loss of airway?
Can result in cardiopulmonary arrest
When would you use an airway adjunct and what are the types?
When GCS<8 and conscious level P or U
Nasopharyngeal airway
Oropharyngeal airway
LMA (laryngeal mask airway)
