Malignant causes of CAO
A
- Extension of adjacent tumor w/ airway invasion->esophageal, laryngeal, thyroid
- Primary intraluminal malignancy->SCC and adenoid cystic carcinoma
- Endobronchial disease
- Compression from a contiguous malignant process

Symptoms
A
- Shortness of breath
- Cough
- Hemoptysis
- Wheeze
- Stridor
In a ventilated patient:
1. +airway pressures, -ve tidal volumes, hypoxemia and hypercapnia

- Lung cancer
- Primary airway malignancy
- Smoking
- Artifical airways
- Tracheobronchial stents
- TE oxygen catheters
- Lung transplant
- NeurocogN/NeuroM disorders
- Granulomatosis w/ polyangitis

Investigations
A
- CXR
Tracheal deviation, mediastinal shift, post-op pneumonia, atelectasis, collapse
? Consider - Bronchoscopy
- CT scan: look for narrowing, masses, SOL, filling defects
- MRI: vascular rings, aneurysms, soft tissue masses
- Spirometry

- Active/past malignancy
- Critical deseases requiring mechanical ventilation
- Previous surgical interventions
- Rheumatological conditions
- Infections: TB, histoplasmosis
- Comorbidities: smoking, diabetes, HTN, high ASA
- Assess aspiration risk

Management
A
1. ABC Establish secure airway 2. High flow oxygen 3. Consider heliox 4. Bronchodilators
