TCP CUBE Trismus (clenched teeth) Croup Patient is able to maintain their own airway. Carcinoma of the upper respiratory tract Unable to perform within 20 secs. Basic techniques are working. Epiglottitis

20 MESSY BOLLOCKS 20 ml syringe Magill forceps ET Tubes (range of sizes) Stethescope Securing tape Yankeur and Suction BVM & Blanket O2 Laryngoscope and spare batteries Laryngoscope Blades (range of sizes) OPA Catheter Mount and CO2 monitor KY jelly Stylet/Bougie
What are the Potential problems for intubation?
BVF CULPRITS Burns to the airway(early intubation recmd) Vomiting/regurgatation Facial Trauma C-spine Abnormalities (flexion restricted). Upper incisers prominant. Long narrow mouth and high arched palate. Patient resists Receeding lower jaw Insecure upper teeth. Trismis Short muscular neck.
Intubation Problems
I LOOK PISTE Intubation of the right main bronchus. Laryngeal Spasm. Occlusion of the cuff against the carina. Oesophageal Intubation. Kinking of the tube. Potentially occlusive reducing airway. Induced Bradycardia/ Inability to perform. Slipping of the tube. Tube blocked with blood or vomit. Equipment Failure
How can you assess the airway?
LEMON Look. Evaluate. (fingers 3,3,2) Mallampati. Obstruction. Neck Mobility.
What are the element of the mallampati?
Class 1 Full visibility of tonsils uvula,soft palate
Class 2 Hard and soft palate, upper portion of tonsils and uvula visible
Class 3 Hard and soft palate, base of uvula
Class 4 Only hard palate visible.
What are the normal ETT sizes
Adult Male 8 - 8.5
Female 7 - 7.5
Paed Age/4 + 4
What are the classes for Mallampati?
