Arndt Airway exchange - has airway adaptor so you can oxygenate while intubating - floppy end goes into suction port & thread
Indications for surgical airway?
Can not intubate, ventilate, wake up, supraglottic has failed, clinically significant hypoxia
IV Catheter/jet ventilation surgical airway?
Access site into cricothyroid ligament, air contrast technique (advance and aspirate until you get bubbles), slide angiocath in& jet ventilate - lot of pressure for small flows. Enk flow modulator allows for passive exhalation & lower flows
Complications of surgical airway with angiocath
Low flow, px of tissue/organ, hemorrhage, subq emphysema, baro/volutrauma, low success rate
Anterograde vs retrogade surgical airway
Anterograde - melker kit, air contrast technique, wire guided, smaller than trach. Retrograde - air contrast but aim up toward mouth, wire comes out through mouth, clamp wire at neck, guide OET over wireto intubate
