how many drusen do you have and what size are they?
if you have a lot of small drusen it will bump things up to another level.
level one to level two
if you have a lot of intermediate drusen it will it
up from a level 2 to a level 3.
pigement abnormalites.
Do you have pigment abnormailits present/absent ?
this is the last thing you look at during the process of grading amd
pigment abnormailites occur in categories 2 and 3.
the geographic atrophy present that is not in the middle bumprs things up to grade 3.
category 1 AMD is
a small drusen and NOT a lot of them (about 5-15) & (less than half the size of the vein at the disc margin)
with no pigment abnormalisits.
category 2 AMD is
more than 15 small drusen,
or
all it takes is ONE intermediate
or
prigment abnormalities by itself.
BUT NO GEOGORAPHIC ATROPHY
category 3 AMD
more than 20 intermediate drusen
or
geographic atrophy in and by itself
or
ONE large drusen. all it takes is one large dursen
Category 4
geographic atrophy in the center circle
or
WET AMD anywhere
or
or VAs reduced secondarily to AMD and advanced amd is not present
or
advanced disease in one eye its not going to matter whats going on in the other eye
engrain this in brain
the small drusen are the size of the CO circle..
the large drusen are the size of C1 circle or more.
if all the small can fit in the O2 circle that is considered intermediate drusen.
on pigment abnormailites
look for
hypopigmentation
or
hyperpigmentation
on grading AMD
dont forget to look for
georgraphic atrophy. this makes it a category 3 or advanced depending on its location!
Geographic atrophy is the
end stage of dry macular degen.
the size of the georgraphic atrophy is directed associated with reading speed.
pseudoreticulum and pseudodrusen has an increased risk of developing geographic atrophy (end stage amd)
