Asymmetrically decreased breath sounds
focal bulla; lung cancer preventing flow distally
Symmetrically decreased breath sounds
– emphysema; someone heavy with thick chest wall
Bilateral High pitched Wheezes
asthma or COPD exacerbation
Focal Wheeze that persists
focal airway obstruction (tumor, aspirated foreign body)
Rhonchi
lots of mucus in airways (bronchitis, pneumonia, COPD, asthma)
Crackles
something in small airways & in/around alveoli (edema fluid, pus, fibrosis)
Pleural Rub
pleuritic from any cause without much pleural fluid keeping pleural surfaces apart
Egophony
consolidative pneumonia; compressed lung at top of large effusion
Localized pain without tenderness
– pleural irritation, airway inflammation if substernal
Localized pain with tenderness
injury to that part of chest wall (eg fracture)
Asymmetrically decreased fremitus
increased diaphragm; anything that keeps pleural surface away from chest wall (effusion, pneumothorax); anything that blocks airflow to that part of lung (endobronchial lesion)
Bronchial breath sounds
a pneumonia with consolidation in that area; top of pleural effusion
