What are the Weiss criteria?
- Fuhrman grade 3-4
- Mitotic rate > 5/hpf
- Atypical mitotic features
- <25% cytoplasm in cells
- > 1/3 diffuse growth pattern
- Necrosis
- Venous invasion
- Sinusoid invasion
- Tumor capsule invasion

What is the chemo drug used for metastatic adrenocortical carcinoma?
What is the survival benefit of this drug?
Mitotane (aka triple D; DDD)
No survival benefit, but tumor does shrink and is less functional
Under what circumstances should you always pursue surgical intervention for an adrenal mass?
- All adrenal tumors > 5cm on CT (b/c CT underest 1 cm on average)
- All functional adrenal tumors
- Suspected adrenocortical carcinoma
