Vasoconstriction, some inhibition of NE (negative feedback loop)

What patient groups are we worried about giving beta 2 nonselective blockers?
COPD patients, asthma patients. Also, diabetics because Beta 2 blockers inhibit insulin, glycogenolysis and gluconeogenesis. After an insulin shot, diabetics then become more prone to hypoglycemia
What is isoproterenol used for?
Vasodilation and increase HR (non-selective beta blocker)
What are the side effects of isoproterenol?
tachycardia, BP, arrhythmias
What is phenylephedrine used for?
Nasal decongestion, mydriatic, hypertensive
What is phenylephedrine used for?
Vasoconstriction
What is clonidine used for?
hypotension, menopausal flushing, migraine
How is clonidine delivered?
IV
What are the effects of clonidine administration?
Acute hypertensive response
What are the side effects of clonidine?
dry mouth, sedation, contact dermatitis
What is dobutamine used for?
Cardiogenic shock
What are the effects of dobutamine?
Increased HR and contractility, dilation of renal and mesenteric vessels
What are the side effects of dobutamine?
Tachycardia, hypertension, hypotension, ectopic activity
What are the side effects of albuterol?
Tachycardia, tremor, tolerance
What is mirabegron used for?
Overactive bladders
What is the action of miraegron? What receptor does it work on?
Beta 3 adrenergic receptor agonist. Relaxes detrusor muscle
What is the side effect of mirabegron?
Tachycardia, HT
What is ephredrine used for?
nasal decongestant, hypotension. Also called pseudoephedrine.
What is the effect of ephedrine?
Increases Norepinephrine release, causing prolonged CNS stimulation.
What are the adverse reactions of ephedrine?
HTN, insomnia, tachyphylaxis
What is amphetamine used for?
Obesity, narcolepsy, ADD
What is the effect of amphetamine?
Increases amine release, causing CNS stimulation
What are the side effects of amphetamine?
Restlessness, tremor, irritability, insomnia, dependency, tolerance
What is tyramine used for?
Increased amine release
What are the side effects of tyramines?
Hypotension…also beware of interaction with MAOIs
What does cocaine do?
Local anesthetic
How does cocaine work?
Inhibits NE, dopamine, CNS stimulant
What are adverse reactions with cocaine?
Hypertension, AMI, arrhythmias, seizures
Name three ways that antagonists can block adrenergic innervation (sympatholytics)
- Decrease sympathetic outflow from the brain
- Suppress nerve terminal NE
- Block post-synaptic receptors
What do you use phentoamine for?
Decreasing blood pressure. It is rarely used.
What do you use phenoxybenzamine for?
Peripheral vasospasm
What are the adverse rxns with phenoxybenzamine?
Hypotension, tachycardia
What is Prazosin used for?
Anti-HTN
What are the adverse effects of prazosin?
First dose effect, large decrease in blood pressure causing postural hypotension
What is propranolol used for?
Hypertension, IHD, arrhythmias
What are the adverse rxns with propranolol?
Bradycardia, CCF, bronchoconstriction, hypoglycemia, aggravated PVD
What do you use timolol for?
Glaucoma
What are the adverse rxns with timolol?
Bronchoconstriction
What is pindolol used for?
Beta blocker used in patients with bradycardia, or with poor cardiac drive
What is sotalol used for?
Atrial and ventricular tachyarrhythmias
What is the effect of sotalol?
Prolongs the cardiac action potential by blocking the K channels
What is metoprolol used for?
HT, IHD
What are adverse reactions associated with metoprolol?
Bronchoconstriction, bradycardia
What is atenolol used for?
HT, renal excretion, IHD
What are adverse reactions associated with atenolol?
Bronchoconstriction, bradycardia
What is labetolol used for?
Hypertension, pheochromocytoma
What are some adverse reactions with labetolol?
Hypotension, tachycardia
What is carvedilol used for?
Treatment of heart failure. It’s been shown to improve survival through antioxidant effects
