Epinephrin

A

Indication: Anaphylaxis, cardiac arrest, bronchospasm
MoA: a and B agonist
Adverse Effects: ARRYTHMIAS, cerebral hemorrhage, anxiety, cold extremities, pulmonary edema
Contraindications: Late term pregnancy
Misc:
Low dose-> lower diastolic BP (B2), increased CO (B1)
High dose -> INcrease TPR and CO (a1+a2+B1) bronchodilation (B2), decreased bronchial secretions (a1)

Norepinephrine

A

Indication: Shock
MoA: a-agonist, B1-agonist
Adverse Effects: Arrythmias, ISCHEMIA, hypertension
Contraindications: PRE-EXISTING VASOCONSTRICTION OR ISCHEMIA, late term pregnancy
Misc: Increase CO (B1), increas TPR (a1+a2), decrease HR (baroreflex), overall increase in MAP

Dopamine

A

Indication: Cardiogenic shock
MoA: B-agonist, some a-agonist activity (D1>D2>B1>a1>a2)
Adverse Effects: LOW BP (at low infusion rates), ISCHEMIA (high infusion rates)
Contraindications: Tachyarrhythmias, v-fib
Misc:
Low rates: decreases TPR (D1), increases CO (B1)
Higher rates: increases MAP and TPR (a1+a2+B1), D1 in renal/splanchnic vascular beds -> protects from ischmia

Indication: Bradycardia/heart block when TPR is high
MoA: B-agonist
Adverse Effects: TACHYARRYTHMIAS
Contraindications: ANGINA WITH ARRYTHMIAS
Misc: Decrease TPR (B2), increase CO (B1), small decrease in MAP, bronchodilation (B2)

Indication: Hypotension during anestheisa, SV tachycardia, mydriatic agent in opthalmic rx, nasal congestion
MoA: a1-agonist
Adverse Effects: HYPERTENSION
Contraindications: Hypertension, v-tach
Misc: Increase TPR and MAP, decrease HR (via baroreflec), pupillary dilation, decrease bronchiole and sinus secretions, no COMT degredation

Indication: Hypertension d/t sympathetic activation
MoA: a2-agonist
Adverse Effects: DRY MOUTH, sedation, bradycardia, HYPERTENSIVE CRISIS (after acute withdrawal)
Contraindications:
Misc: Acute increase in BP (peripheral effect), reduced BP (central effect), a2 receptors in CNS on sympathetic neurons, inhibit these and reduce sympathetic drive

Misc: Less bradycardia and lipid abnormalities, less severe side effects
