Background information to establish

A

History of ADR or allergies
Timing of reaction in relation to start or dose increase of suspected drug
Get full description of signs and symptoms of reaction - clarify reactions such as rash or abnormal LFTs or aching all over for e.g.
Has suspected drug been stopped
How has patient managed so far
Has rechallenge, deliberate, or inadvertent been undertaken
Did suspected ADR resolve when suspect drug was stopped
Results of biochemical tests
Does enquirer want to know which drug is likely to have caused it
Enquirer involved in legal case?
Has manufacturer been informed or yellow card completed

Prospective enquiries

A

Does enquirer think patient may be at particular risk or an ADR e.g., patient with history of ADR to same class of drugs - if so what were signs and symptoms of suspected previous reaction
Does enquirer just want general information
Does enquirer want to know more information about a specific side effect
Does enquirer want to assess risk/benefit comparison between 2 drugs e.g., which is safer often in relation to specific side effect
Some enquiries can involve both - patient had reaction thought to be ADR and enquirer wants suggestions for alternatives
