what is the purpose of admission?
q- verify the patient’s identity (ask for name+ IC number/ DOB)
- assess his clinical status, make his as comfortable as possible
- introduce him to his roommates and staff
- orientate him to the environment and routine
- provide supplies and special equipment needed for daily care
why are admission routines so important?
Admission routines that are efficient and show appropriate concern for the patient and ease his anxiety and promote cooperation and receptivity to treatment. Conversely admission routines that the patient receives as careless or excessively impersonal can heighten anxiety, reduce cooperation, impair his response to treatment, or perhaps aggravate symptoms.
what are the sources of admission?
- emergency department (ED)
- specialist outpatient clinic (SOC)
- hospital admission office
- inter-ward transfer
- other wards/ institutions
what are some modes of admission?
- walk-in/ carried (infants)
- wheelchair
- trolley/bed
what are some types of wards?
- disciplines (eg. oncology, surgical, medical, neurology)
- class A, B, C
what are some types of patients?
- resident, non-resident
- industrial accident
- work permit/ employment and other pass holders
- police cases
- VIPs
what are some payment forms?
- deposit
- letter of guarantee

what is the nurses’ responsibility for admission?
- if a patient has a sensory/ communication need
- if they require any assistive devices (wheelchair, walking/hearing aid)
- screen all patients on admission to a healthcare setting for possible discharge needs (CGT aka Caregiver Training, PFE aka patient family education) to ensure that appropriate teaching is completed (might need immediate caregiver)
- include patient, family and relevant healthcare professionals (PT, OT, ST) early in planning to promote successful transition through the healthcare system
- consider a patient’s education background, health literacy level, and ability to understand instructions
what are included when taking vital signs?
- temperature, pulse, respiration (TPR), Bp, SpO2, pain score (PS)
