- You are the captain at Fire Station 473. FF Jones comes to you and tells you that during her morning check she noticed a large dent and scratches on the drivers side of the medic unit.
a. Evaluate damage to vehicle. Can it stay in service?
b. If no, contact BMT and UFO and place unit OOS. Contact duty apparatus and arrange to have the vehicle evaluated and/or switch over
c. If vehicle can remain in service, contact BMT and SAFO to notify of findings
d. Check Pass-on book and station logbook. Check past online vehicle inspections
e. Contact officer of other shifts to see if he has any information. If not, contact medic and medic driver from other shifts.
f. Have FF Jones document what she found and the circumstances leading up in a FRD-317
g. Complete FRD-072 and forward to BMT and SAFO
h. Make notation on pass-on book and log book.
i. Notify Station Commander
j. Review SOP 03.06.01 on vehicle accidents with the shift and ensure all personnel know the proper process for reporting any accidents or finding of damage during morning checks. Also review SOP 03.06.06 on acquiring reserve apparatus if appropriate
You are the captain at Fire Station 473. You overhear FF Smith asking Lt. Brown if he can ride the truck next day. Lt. Brown tells FF Smith that he will never be allowed to ride the truck because he is “a retard”.
a. Stop the interaction between the two employees and separate them.
b. Place the unit they are on out of service. Attempt to arrange staffing to have the minimal effect on operations
c. Notify UFO of units OOS and notify BMT of units OOS, staffing adjustments, and issue.
d. Notify Lt. Brown that his comments were unacceptable, contradict the departments core value of teamwork, Rules and Regs 100.06 Human Relations and 200.19 conduct unbecoming and the departments zero tolerance policy on harassment. Ask Lt. Brown to compose a memo stating the circumstances leading up to his comments.
e. Inform FF Smith that the county as a “zero-tolerance” policy on harassment. If FF Smith feels he has been harassed, inform him he can file a complaint with OHREP by calling 703-324-2953 or with EEOC at 800-669-4000.
f. Ask FF Smith to compose a memo with his account of what occurred.
g. See if anyone else witnessed or heard the incident and solicit memos from them if they witnessed anything.
h. Check FF Smith and Lt Brown’s Personnel file for any history of similar incidents.
i. Check to see if FF Smith is “TQ” on Telestaff or if he has the appropriate records of completed Truck Task book and FRD-436 Telestaff Update
j. Forward Memos and observations to BC. Recommend progressive discipline as appropriate.
k. Hold a meeting with the shift to brief them on Human Relations, conduct unbecoming, and harassment.
l. Explain to FF Smith the requirements to ride the truck, inform to him how he can go through the process, and offer mentoring to enable him to ride the truck.
m. Update personnel files and make supervisory notes
FF Harris informs you that she has just learned that she is pregnant and she isn’t sure how this will impact her career. She comes to you looking for advice on how to handle the matter
a. Advise her to inform PSOHC as soon as her pregnancy is confirmed
b. Ensure she has a copy of the Pregnancy and Reproductive Manual so she can be aware of her rights, and pertinent information to make informed decisions
c. Inform her that it is her right to make her own decision regarding work status duing pregnancy
d. Inquire if she would like to continue working in the field or would like a light-duty assignment
e. Have her complete a Work Status Notification (FRD-158) and contact the Return to Work Program Supervisor for light-duty assignment per S.O.P 02.03.06
f. Ensure she completes FMLA paperwork and submits it. S.O.P 02.01.12
g. Put her in contact with the Wellness and Fitness program to maintain fitness during her pregnancy
