Who is normally responsible for notifying the GMPO/Site VP?
Admin 4.0 directs it however;
The Duty Station Manager is normally responsible for notification of the Site Vice
President and General Manager Plant Operations, unless he/she feels unable to
purvey the message due to the technical content.
Under what condtions/events does the NRC Resident get notified?
- Threats to Generation - Any event or equipment malfunction that could threaten plant reliability or availability (loss of greater than 25 MWe) within the next 72 hours.
- Entry into the emergency plan by declaration of Notification of Unusual Event, Alert, Site Area Emergency or General Emergency.
- Entry into AOP or EOP
- Threats to Security
- Violation of any Technical Specification safety limit.
- Any events requiring emergency response to the Plant site, including Fire Department, Ambulance Service, or local law enforcement.
- Reactor Trip.
- REPORTABLE EVENT (Refer to EN-LI-108 and REAP program) that requires notification in 72 hours or less.
Who should the shift manager consult with on difficult reporting determination issues?
The Shift Manager is encouraged to contact members of the Licensing
Department to discuss the reportability of difficult issues.
What are the two forms for NRC Operations Center notifications?
Non-Emergency: NRC Form 361, “Event Notification Worksheet,”
Emergency: Emergency Implementing Procedure EI-3, “Communications and
Notifications,” Attachment 1, “Emergency Notification Form.”
Are all non-emergencies reported on Non-Emergency: NRC Form 361, “Event Notification Worksheet,”?
No. EI Procedures classify some as an emergency and they SHALL be reported using Emergency Implementing Procedure EI-3, “Communications and
Notifications,” Attachment 1, “Emergency Notification Form.”
What happens to completed Non-Emergency and Emergency Notification Forms?
Should be forwarded to the Licensing Administrator as soon as
practical
If Big Rock reports out a medical injury of a contaminated person - 8 hour NRC report, what is required of Palisades SRO?
The expectation is that a Palisades SRO contact a member of Palisades Licensing Department for initiation of the event report within the eight (8) hour time frame.
If the MET Tower top flashing light fails, what do you do?
Immediately reported to the Federal Aviation Administration (FAA) (1-877-487-6867) when the failure is noted and when it is corrected. The failure notification is called a “Notice to Airmen” (NOTAM).
Failure notification must be repeated if light(s) not repaired within
15 days.
When is the NRC notified of issues impacting the environment?
Within 24 hours of Any occurrence of an unusual or important event that indicates or could result in significant environmental impact whose cause could be or is related to Plant operation.
Examples:
- Excessive bird impaction events,
- Onsite plant or animal disease outbreaks,
- Mortality or unusual occurrence of any species protected by the Endangered Species Act of 1973, fish kills,
- Increase in nuisance organisms or conditions
- Unanticipated or emergency discharge of water or chemical substances.
Who do you contact if NPDES limit may have been exceeded?
A Licensed Waste Water Operator shall be notified in accordance with
“SPCC/PIPP Oil and Hazardous Material Spill Prevention Plan,”
When do you contact a Licensed Waste Water Operator?
- Prior to bypassing or removing from service either Waste Oil Separator M-18 or Turbine Building Drain Tank, T-41.
- Anytime there is reason to suspect that a NPDES limit may have been exceeded.
The Fire Protection Section gets notified for what?
Whenever any portion of the Fire Suppression System is out of service or any portion of the Fire Detection System might be out of service for two or more hours.
Who gets nofified and how for use of steam dumps and other unusual activities onsite of which the public may become aware through sensory perception?
The Van Buren Sheriff and Public Affairs shall be notified using NRC Form 361, “Event Notification Worksheet,
NRC notification of steam dump use is not required.
When a derate and hold at less than 98% power is required for greater than 24 hours, what do you do?
Notify the Engineering Duty Manager to have PRA perform the following within 24 hours.
- Perform a Risk Assessment/Analysis.
- Align with Operations Management on any needed defense in depth measures that may need to be implemented for the duration of the condition.
Submit a Condition Report to have Steps i.1.(a) and i.1.(b) above completed and documented.
This does not apply to normal maneuvering transient time such as during a start up to full power. This does apply to Chemistry holds which are predicted to last more than 24 hours.
When are asymmetric loads on PCS internal components greatest?
Palisades LOCA asymmetric loads analysis for the PCS internal components was
performed at hot full power conditions (nominal 98% power). Stresses are greater
at lower powers. (Reference CR-PLP-2012-01125.)
