What are the core symptoms of ADHD?
Inattentiveness
Hyperactivity
Impulsivity
What screening tools are used to diagnose ADHD?
SNAP-IV:
Score above 5% cut-off is highly indicative of ADHD
Conner’s Rating Scale:
T-score <60 = no ADHD
T-score >60 = may have ADHD
T-score >70 = more serious ADHD symptoms
The Qb Test:
Measures inattention, hyperactivity and impulsivity
A Q score between -1 and 1 is normal
A Q score between 1 and 1.4 is abnormal
In activity cross, ADHD children will have large messy graphs with dense markings
In attention and impulsivity graph, ADHD children will have more incorrect answers, more anticipatory errors and more omission errors (missed clicking)
What is the management for ADHD?
Education
Behavioural management
Support for teachers
Medication:
Used as a last resort and only in children aged 5 and over
1st line = methylphenidate
2nd line = lisdexamfetamine
If benefitting from lisdexamfetamine but can’t tolerate side effects, give dexamfetamine
In adults, methylphenidate or lisdexamfetamine are 1st line
All these drugs are cardio toxic, therefore take baseline ECG prior to starting.
Also monitor weight, height and BMI in children every 6 months
Side effects:
Abdominal pain, nausea, dyspepsia
Appetite suppression
Insomnia
