Class
Antidysrhythmic
Mechanism of Action
Slows AV node conduction, can interrupt re-entry pathway, decrease HR by acting on sinus pacemaker cells, Drug used for re-entry SVT, diagnostic use for stable wide complex-tachycardia post 2 doses of lidocaine
Indications
Conversion of PSVT to sinus rhythm, Wolf-Parkinson White Syndrome, most stable narrow complex SVT, not effective for a fib/flutter conversion
Contraindication
2nd or 3rd degree AV block (if no pacemaker), sick-sinus syndrome (if no pacemaker), asthma, COPD, drug induced tachycardia
Averse Reactions
HA, dizziness, dyspnea, bronchospasm, dysrhythmias, palpitations, hypotension, cpx, flushing, cardiac arrest, nausea, metallic taste, paresthesia, diaphoresis
Drug Interactions
Theophylline-like drugs have antagonistic effects, Dipyridamole potentiates effect, Carbamazepine potentiates AV node blocking effect
How Supplied
3mg/ml, in 2ml or 5ml vials
Dosage and Administration
Adult: 6mg rapid IVP bolus (1-3 sec), followed by 20ml flush, elevate extremity. May repeat after 1-2 min with 12ml IVP bolus (1-3 sec)
Ped: .01 mg/kg IVP/IOP (max 1st dose 6mg) followed by 5-10 ml flush. May repeat 2nd dose of .2mg/kg IVP/IOP followed by flush

Special Considerations
Pregnancy: Cat C
Bronchoconstriction in asthma pts, Evaluate elderly pts for dehydration requiring fluid replacement prior to administration
