A
  • PEEP 5-18 cm H2O
  • increase inspiratory times (increases intrinsic PEEP)
  • small TV, < 7 ml/kg
  • reduced FiO2, < 50% asap; acceptable hypoxemia (SaO2 > 88%)
  • permissive hypercapnia
  • +/- prone position
