common skin disorder usually occuring in adolescence
characterised by
obstruction of pilosebaceous follice with keratin plugs which results in comendones, inflamation and pustules
pathophysiology
- follicular epidermal hyperproliferation –> keratin plug which obstructs follicle
- colonisation by anaerobic bacterria Propionibacterium Acnes
- inflammation
mild acne
open and closed comeodnes
with or without sparse inflammatory lesions
moderate acne
widespread non-inflammatory lesions and numerous papules and pustules
severe acne
extensive inflammatory lesions
may include nodules, pitting and scarring
1st line Mx
single topical therapy
- topical retinoids
- benzoyl peroxidase
2nd line Mx
topical combination therapy
- topical antibiotic
- topical retinoid
- benzoyl peroxidase
first line oral antib
tetracyclines
when should tetracylcines be avoided
pregnanct/breastfeeding women
kids < 12yrs
what oral antib can be used in pregnancy for acne
erythromycin
Mx - what can be considered as alternative to oral antib in women
COCP
what must be prescribes along with oral antib
topical retinoid and benzyl peroxidase
reduce risk of antib resistance
